New Hampshire’s rural northern counties face primary-care physician ratios of 1:3,200, more than double the state average, compounded by an aging population where 22 percent of residents are over age 65. Limited broadband in the White Mountains region restricts telehealth readiness for outreach programs.

These constraints affect nonprofit health organizations and municipal health departments operating in Coos and Grafton counties. Many rely on seasonal tourism workers who lack year-round insurance, creating episodic demand spikes during ski and foliage seasons.

Infrastructure limitations include only two Federally Qualified Health Centers north of Concord, forcing outreach teams to travel on secondary roads that close during winter storms. Workforce shortages are acute: the state’s nursing vacancy rate exceeds 15 percent, leaving few staff available for community workshop delivery.

The Community Health Outreach Programs grant can support mobile clinic retrofits and satellite internet subscriptions required for real-time eligibility screening. Funds must also cover winterized vehicle leases to maintain service continuity.

Unlike Vermont applications, New Hampshire proposals must document formal agreements with the state’s 10 regional public health networks and demonstrate compliance with the New Hampshire Charitable Foundation’s outcome reporting template.

Infrastructure and Workforce Constraints
